ADMISSIONS AND RE-ENROLLMENT FORMS

APPLICATION FOR ADMISSION TO FAITH LUTHERAN PRESCHOOL:

To-Do List:

Make an appointment for an interview with the Preschool Assistant Administrator. When a student is accepted, the following must be submitted to the office before student is allowed to attend classes:

  • A non-refundable registration fee (see Fee Schedule). A discounted registration fee is offered for early registration.
  • Immunization Records (or a Medical/Health Information form signed by your child’s physician)
  • Birth Certificate (copy)
  • Medical/Health Information Form with Doctor’s Signature and date of most recent well-child exam.
  • New Student Application Forms (complete online, print, sign and bring to school office).
  • Simply Giving Member Enrollment and Authorization Form with voided check (or tuition payment) submitted to school office (complete online, print, sign and bring to school office).

RE-ENROLLMENT IN FAITH LUTHERAN PRESCHOOL:

The following must be submitted to the office before student is re-enrolled:

  • A non-refundable registration fee (see Fee Schedule). A discounted registration fee is offered for early registration.
  • Immunization Updates.
  • Medical/Health Information Form with Doctor’s Signature and date of most recent well-child exam. Returning students are required to return the medical form only after the next annual well-child exam.
  • Re-enrollment Forms (complete online, print, sign and bring to school office).
  • Simply Giving Member Enrollment and Authorization Form with voided check (or tuition payment) submitted to school office (complete online, print, sign and bring to school office).

APPLICATION FOR ADMISSION TO FAITH LUTHERAN SCHOOL (KINDERGARTEN – 8TH GRADE):

To-Do List:

Make an appointment to interview the School Administrator and schedule a testing time for your child. Bring your child for testing along with the following:

  • A non-refundable testing fee (see Fee Schedule).
  • A copy of your child’s report cards, standardized test scores and any other specialized testing.
You will be notified by the Administration concerning the student’s acceptance no sooner than 24 hours after testing.

EXTENDED CARE ENROLLMENT:

The following must be submitted to the office before student is enrolled:

  • A non-refundable registration fee of $50.00 for Preschool students
  • Or a non-refundable registration fee of $100.00 for K – 8th grade students.
  • Extended Care Enrollment
  • Simply Giving Member Enrollment and Authorization Form completed and signed
